Nurse Case Manager for Suboxone and Pain Management

The Nurse Case Manager for Suboxone and Pain Management provides nursing services to patients in accordance with all applicable Massachusetts State Laws as well as regulations promulgated by the Department of Public Health and Harbor Health Services, Inc. Duties include but are not limited to the following services for pain management and substance abuse/mental health patients: triage; assessment; case management; education/training, outreach, tracking, reporting and monitoring of related data. This position requires successful completion of required training, and an independent level of assessment skills and responsibility for case management.

Minimum Skills, Experience and Educational Requirements:

5 years of experience in a professional or community health center setting, 1 year of experience in addiction or behavioral health, excellent customer service skills, excellent verbal and written communications skills, Basic computer literacy,

ability to effectively work within both a team setting and independently, ability to work effectively with economically, racially and linguistically diverse customers.

Required Certifications and Licenses:

Current Massachusetts Registered Nurse, Certification of Suboxone training or obtained within one year of employment.

Substitutions for Skills, Experience and Education:

BS in Nursing for one year of experience.

Preferred Skills, Experience and Educational Requirements:

4 or more years of experience in a substance abuse or behavioral health setting,

Experience in a community health setting, experience in an ambulatory setting, experience in pain management.
